To complete your payment and registration, please use the following instructions to pay by cheque: 

  1. Make cheques or bank drafts payable to “Surrey Place”  

2. Mail your cheque to: ATTN: Finance Department, Surrey Place, 2 Surrey Place, Toronto, ON, M5S 2C2  

Please note that Canada Post is currently on strike. We encourage you to either drop off your cheque in person (see available locations) or select another shipping provider.

3. Important: Please ensure you write your child’s name and program name on the cheque. (Please include your child’s Surrey Place Client ID/SPID, if you have one.)  

You have two options:    

Payment schedule: To secure your spot, a $1,000 deposit is required within 48 hours of receiving this email. Once your deposit is received, your payment schedule will be as follows: 

OAP Receipts: Please note that once your payment has been received, you will be issued a receipt. However, once the service has started, we will issue a separate OAP invoice, which can be used to reconcile your Access OAP funding.
