Service Coordination: What to Expect

WHAT IS SERVICE COORDINATION?
Service coordination is a goal-based service and duration depends on the needs of the client and family. A service contract will be created at the onset of service outlining the identified goals and service duration.
A Service Coordinator is assigned to assist a parent/caregiver in accessing services in the community as well as at Surrey Place. The Service Coordinator can liaise with the school, daycare and other community supports such as funding, resources, programs, and respite.
When you come in for your service coordination appoint, you can expect the following:
A service coordinator is assigned to a client and will contact the family member or caregiver to set up an initial meeting. This can be done over the phone or in person.
Service coordination can be a short consultation or longer intervention depending on the needs of the client and the family. Meetings are scheduled as needed.
First contact is often for introductions, confirmation of address, phone numbers, date of birth and diagnosis.
A discussion around services, goals and areas of need are identified.
Discussion about all other clinical or medical services that the client is accessing or waiting for. Additional reports, consents and documents may be requested.
Review of all other agencies involved and contact information.
Identify funding being accessed and identify needs for additional funding.
Discussion of daycare, school or other day programs
Review of any respite, recreation or camp being accessed or required.
If applying for income based financial assistance and/or funding copies of the following may be requested:
Notice of Assessment for each parent
Birth Certificate for Child and Parent(s)
Child’s Health Card
SIN
If not born in Canada, immigration documentation for parent(s) (PR, Citizenship)
Assessment or Diagnostic Report
CCB Statement
Void Cheque or Direct Deposit Form
Two Passport Sized Photos
