Eligibility Criteria
• Confirmed diagnosis of Down Syndrome
• 0-6 years of age
• Toronto resident living in M postal code
How to Refer
• For new or current clients of Surrey Place, request a medical referral letter from your primary care provider
• Please fax the referral to 416-929-8199, with attention to: Down Syndrome Consultation Clinic.
